Welcome to our office! New patient consults are typically one hour long. This time will be spent doing a full history and review of any past records, lab work and imaging studies you may have had done (please request these records in advance so they will be here in time for your appointment). Diagnostic tests will then be ordered and a follow up appointment will be made to review the results and make a treatment plan.
Prior to checking in, we will need necessary information to help you plan your care, there are a number of forms you will be asked to fill out and return to our office. If you are unable to print these forms, please contact us so we can mail them out to you. Please arrive a few minutes prior to your appointment time for our staff to review and make sure everything is completed as needed.
Our insurance specialist will review your insurance information and verify the coverage you have when you arrive, so please be prepared to show your insurance card and picture I.D. Fertility treatments may be considerable, so it is advisable that you call your plan and get an understanding of your benefits before your appointment.

We are located in the Providence St. Vincent Hospital complex in the East Pavilion,
Suite 320. If you are driving, park in the East Pavilion parking structure. If using public transportation, the #20 bus stops in front of the building. The closest Max stop is the Sunset Station and there is a free shuttle bus (every 20 minutes) to St. Vincents (or it is a 10 minute walk).
